Park Avenue Surgery works together with your surgeon and their team to make sure you have a safe and comfortable experience. Please review the information below carefully so you, your friend, family member, and your surgeon and their team can work together to give you the best possible care and surgical outcome.

If You Need To Schedule or Cancel Your Surgery

To schedule a procedure, cancel your procedure, schedule a follow-up on results, schedule a post-operative visit or if you have post-operative questions, please contact your surgeon’s office.

If you have a medical emergency please call 911 or go to the nearest emergency room.

Medical Clearance, Bloodwork, Imaging and EKG

If requested by your surgeon, please make sure you get bloodwork, medical clearance, imaging and/or EKG within 30 days of your procedure. Make sure to schedule your appointments with time to get the results back, as your surgeon’s office will be sending your medical files to Park Avenue Surgery a week before your scheduled surgery.

Covid-19 Pre-Screening and Pre-Surgery Covid-19 Testing

You must provide proof of full COVID-19 vaccination (last dose not less than 14 days from the date of your surgery) or no sooner than five days prior to your scheduled surgery, you must get a COVID-19 PCR test. Antigen testing and antibody testing are not authorized testing options. Your surgeon may offer PCR testing in their office or can refer you to a facility. If you have trouble finding a testing location, Park Avenue Surgery offers COVID-19, pre-surgical rapid PCR testing for $395.

The day before your scheduled surgery, a member from the Park Avenue Surgery team will call you and conduct a COVID-19 pre-screening interview.

Upon arrival at the facility, you will have another COVID-19 screening interview.

The COVID-19 screening questions are:

  1. Have you and/or anyone accompanying you traveled outside the U.S.A. in the past 10 days?
  2. Have you and/or anyone accompanying you been in close contact with a person known to have COVID-19 in the past 10 days?
  3. Do you and/or anyone accompanying you today currently have a fever or any respiratory symptoms such as a cough or shortness of breath?
