Park Avenue Surgery is committed to providing the highest level of quality surgical care and personalized service to our patients. We work closely with your surgeon’s office to provide you with the most accurate fee estimate for your procedure. The fees for your surgical procedure will consist of three to four components:

1. Surgeon’s fee
2. Facility fee
3. Anesthesia fee
4. Pathology Laboratory fee (if applicable)

Your estimated facility and anesthesia fees are based on the time required to perform your surgery.

Your Surgeon’s Fee

This fee is from your surgeon’s office and covers the price of your surgery and any implants. You pay this fee directly to your surgeon. Park Avenue Surgery does not collect payment on behalf of your surgeon and is unable to answer any questions regarding this fee. Facility Fee (Park Avenue Surgery)

This fee covers the utilization of Park Avenue Surgery for your procedure. It also covers the supplies, medications and staffing needed to provide you with a safe surgery. The facility fee is based on time in the facility, including preoperative time, your surgeon’s operating time and recovery time. Our business team will contact you approximately one week prior to your surgery to collect your estimated portion of the facility fee. The amount collected is an estimate only. In some instances, there are circumstances in which your surgery may last longer than anticipated and your facility fee will increase.

Payments can be made by credit card or debit card. We also accept CareCredit and have financing available to those who qualify.

Anesthesia Fee (Caliber Anesthesia)

Park Avenue Surgery is partnered with Caliber Anesthesia to provide the highest standard of anesthesia care to our patients. Caliber Anesthesia is a separate provider from your surgeon’s office and Park Avenue Surgery. The anesthesia fee is based on the time the anesthesiologist is used, including pre-surgical sedation time, your surgeon’s operating time and recovery time with the anesthesiologist. A team member from Caliber Anesthesia will contact you approximately one week prior to your surgery to collect your estimated portion for anesthesia. The amount collected is an estimate only. In some instances, there are circumstances in which your surgery may last longer than anticipated and your anesthesia fee will increase.

Pathology

We send pathology to a designated facility if requested by the surgeon.

If your procedure requires a specimen or culture to be collected for review, Park Avenue Surgery will send your insurance information to the pathology laboratory. In most instances, pathology is a covered benefit. However, in the event that the laboratory does not take your insurance, you have a deductible or copayment, or the pathology is considered a non-covered service, the laboratory may send you a bill after your specimen has been reviewed.

Park Avenue Surgery does not bill insurance for cosmetic and non-medically necessary procedures.

Your surgeon’s office and Park Avenue Surgery make every effort to accurately estimate your facility and anesthesia times. Should the actual facility time and/or anesthesia time exceed what was estimated, Park Avenue Surgery and/or Caliber Anesthesia will contact you regarding the additional fees.

Park Avenue Surgery does not bill for the services of your surgeon, Caliber Anesthesia or any other outside services.

Patients have the right to request information and ask questions about any fees or payments. If you have questions regarding your surgical fee, please contact your surgeon’s office.
